Before the start of the school year, parents of schoolchildren in Dnipro must complete two documents: a medical certificate form No. 086/o and a vaccination record form No. 063/o. The certificate is issued by a pediatrician or family doctor after an examination – it records the child's health status, the doctor's conclusions, the date of the next examination, and the group for physical education classes. Form No. 063/o reflects all vaccinations made in accordance with the National Vaccination Calendar.
According to current legislation, a child without mandatory vaccinations cannot attend in-person classes – unless there is an official medical contraindication. If a child transfers to another institution or receives a new vaccination, the card needs to be updated. Parents are advised to contact their family doctor or pediatrician in advance to avoid queues.
The state of martial law does not cancel or change the procedure for medical examinations for education workers, emphasize the State Service of Ukraine for Food Safety and Consumer Protection in Dnipropetrovsk Oblast. The requirement applies to everyone without exception – from managers and teachers to educators, medical workers, food service staff, and technical services. Medical examinations are a condition for admission to work and are aimed at preserving the health of both staff and children.
The organization and financing of examinations are the employer's responsibility.
The organization and conduct of mandatory medical examinations are provided by the employer at their own expense. Employees who, without valid reasons, have not undergone an examination within the established period may be suspended from work, – note the specialists of the State Service of Ukraine for Food Safety and Consumer Protection in Dnipropetrovsk Oblast.
Examinations for administration, teachers, and technical staff
The list of examinations depends on the employee's category – in accordance with the requirements of the Ministry of Health of Ukraine, effective from 2024. For administration, teaching, and technical staff, the following are provided:
examination by a dentist and an ENT specialist – during the preliminary medical examination;
annual examination by a therapist and a dermatovenereologist;
blood test for syphilis;
testing for intestinal infections and helminthiasis.
Fluorography during preventive examinations is performed only if there are symptoms, diseases, or risk factors.
Requirements for medical workers and food service staff
Medical workers of educational institutions additionally undergo testing for hepatitis C, and during the preliminary examination – testing for hepatitis B, if there is no confirmation of vaccination. The list for food service staff is broader: in addition to standard tests, they submit tests for typhoid fever, paratyphoid A and B, salmonellosis, and shigellosis, as well as a nasal swab for Staphylococcus aureus. Hepatitis A is checked in the absence of vaccination data.
According to the updated rules of the Ministry of Health 2024, a psychiatric examination for employees of educational institutions of all forms of ownership is conducted once every five years – previously it was conducted annually. The examination includes laboratory testing for psychoactive substances. If the employer has suspicions about the employee's mental state, they can initiate an unscheduled examination, but only with the employee's consent; during the examination period, the person retains their average earnings.
